Cystinosis nephropathy, or nephrosclerosis in cystinosis, is a severe autosomal recessive metabolic disorder where cystine accumulates within lysosomes in kidney cells. This progressive buildup primarily affects renal tubules, initially causing Fanconi syndrome with electrolyte imbalances, polyuria, and growth failure in infancy. Over time, sustained cystine accumulation leads to irreversible scarring and hardening of kidney tissue (nephrosclerosis), severely impairing filtration and excretory functions. Untreated, this ultimately progresses to chronic kidney disease and end-stage renal disease (ESRD), often necessitating dialysis or kidney transplantation by adolescence. Lifelong cysteamine therapy slows progression but cannot reverse established damage.
