Persistent high blood flow to the lungs causes irreversible pulmonary hypertension, eventually reversing the shunt direction (right-to-left). This results in deoxygenated blood entering systemic circulation, causing cyanosis, clubbing, and polycythemia. Symptoms include shortness of breath, chest pain, and fatigue. Without intervention, it leads to progressive heart failure, arrhythmias, and significantly reduced life expectancy.
